During running 4 1/2” tubing, single joint elevator was found installed in reverse position – unsafe condition, potential for equipment damage or pipe drop.
Action Taken:
Stopped the job, lowered tubing, and reinstalled single joint elevator correctly.
Job resumed safely. |

Found that the shaft of the lock open clip of the airlock door from Accommodation to maindeck has been loosen and fell out. That make the door not working properly, the latch of the door will be damaged very soon because of impact. I came to the mechanic shop and ask them to fix the shaft to make the lock open clip work properly. They agreed and fix the problem together. |
