I noticed a mismatch between the tubing coupling and the pumping sub crossover when the rig crew attempted to make the connection.
I stopped the operation and informed the crew that the connection was incorrect and needed to be replaced with the correct one.
Ensure proper type of connections between components before making up to prevent equipment damage or potential safety hazards. |

During tubing hanger laydown from work basket. I observed a man standing and working under a suspended load.
I immediately stopped the job and instructed the man to move out of the danger zone.
No one should ever work or stand under a suspended load. All personnel must stay clear of suspended objects to prevent potential drop objects. |
