I noticed the pin bolt on the slip was at the end and almost out. I told the jack operator to tighten it again, as it could cause the bolt to come loose and fall into the hole or the slip didnt sit properly when closed position. I was worried the string would sag, especially since the string wasn't heavy enough. |

During lifting operation to lay down ESP Wooden Box from Work Basket to Main deck, the crew from PVEPCL not involve the job go inside around the lifting area. Someone put the hand signal to stop him to wait but he takes out the hand signal and continue entrance the area during the load does not finish. |
